How to request your first appointment:

1.                   Fill out the form below with the requested information 
          (or you may call me at 469-0146 if you prefer);
 
2.                 I will phone you promptly after I receive your email request;

3.                 During our phone call, I will ask you some very brief questions 
          about why you are coming to counseling and to ensure that as a 
          Licensed Professional Counselor, I can offer you the appropriate 
          services that you need.
 
4.                 We will schedule an appointment or I will give you a referral 
          to an appropriate professional;
 
5.                 I will then email you the initial paperwork that you can complete 
          at your convenience and bring with you to our first session.
